ANTIHYPERTENSIVE DRUGS

Ambrisentan falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Ambrisentan (Letairis): this medicine relaxes and opens up the blood vessels in your lungs, and along the way it can also nudge your overall blood pressure down. Danshen, in lab and animal studies, seems to lower blood pressure too. So the worry is simply additive. Stacking two things that both relax blood vessels could leave your pressure lower than expected.
What might that feel like? Dizziness, lightheadedness (especially when you stand up), headache, or feeling unusually tired. I'll be honest, this concern is theoretical and comes from animal data, not strong human studies, so don't panic. Still, since you're managing PAH, it's worth a quick chat with your pharmacist or doctor before adding Danshen.

P-GLYCOPROTEIN SUBSTRATES
2 of 3Ambrisentan also falls under P-GLYCOPROTEIN SUBSTRATES.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist: ambrisentan is moved around your body partly by a transporter called p-glycoprotein, and Danshen seems to slow that transporter down a bit. In a small study, Danshen pushed up the levels of another p-glycoprotein drug by roughly a third. So the theoretical concern is that Danshen could nudge your ambrisentan levels a little higher than expected.
If that happens, you might notice more of ambrisentan's usual effects, like swelling in your ankles or feet, flushing, a stuffy nose, or feeling more tired or dizzy. None of this is a sure thing, and the evidence is fairly limited. Still, since Danshen can also affect bleeding, it's worth a quick chat with your pharmacist or doctor before combining the two. Don't change anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Ambrisentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the connection: your body uses an enzyme called CYP3A4 to help clear ambrisentan, and Danshen can nudge that enzyme in either direction. Taken regularly for a week or two, Danshen seems to speed CYP3A4 up, which could lower your ambrisentan levels and make it work less well. Since this medicine is helping control the pressure in your lung vessels, weaker effect could mean more shortness of breath or less stamina over time.
The flip side: a single dose of Danshen may briefly raise drug levels instead, which can mean more swelling, headache, or stuffy nose. The human evidence here is early and modest, not a sure thing. Ambrisentan is a real-deal drug for a serious condition, so please loop in your pharmacist or doctor before adding Danshen.

What Is Ambrisentan Used For?
Brand names: Letairis, Volibris
Ambrisentan tablets and Letairis tablets are both used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — a condition where the blood pressure in the arteries leading to the lungs is dangerously high. Both are approved to help adults exercise better and slow the worsening of PAH.
Both ambrisentan tablets and Letairis are also approved to be used together with tadalafil (another PAH medicine) to further reduce the risk of disease progression, lower the chance of hospitalization for worsening PAH, and improve exercise ability. Most of the clinical evidence was in patients with mild-to-moderate symptoms and PAH caused by unknown or inherited causes, or PAH linked to connective tissue diseases.
