Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Vitrakvi (larotrectinib): your body relies on a liver and gut enzyme called CYP3A4 to break it down. Danshen can nudge that enzyme in either direction. Taken steadily for a week or two, it seems to speed up CYP3A4, which could clear larotrectinib faster and weaken it, and we really don't want a weaker cancer medicine. Oddly, a single dose of danshen does the opposite and can push levels up, which could mean more side effects like dizziness, fatigue, nausea, or constipation.

This is from small studies in healthy people, not from people on this exact drug, so it's a real possibility but not a sure thing. Larotrectinib is too important to gamble with, so please check with your oncologist or pharmacist before adding danshen.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adults and children with solid tumors — meaning cancers in organs or tissues — that have a specific genetic change called an NTRK gene fusion. To be eligible, the tumor must be metastatic (spread to other parts of the body) or be one where surgery would likely cause serious harm, and other treatments must have stopped working or not be available.

Patients are selected for Vitrakvi based on an FDA-approved genetic test that confirms the NTRK fusion in their tumor. This approval was granted under the FDA's accelerated approval program, based on how well tumors responded to treatment; continued approval depends on results from ongoing confirmatory trials.
