Why this applies to Larotrectinib

Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Vitrakvi (larotrectinib): your body relies on a liver enzyme called CYP3A4 to break this medicine down. Lentinan, in early lab research, seems to speed that enzyme up. If that happened in a real person, your body could clear larotrectinib faster than intended, leaving lower levels in your blood. Since larotrectinib is an active cancer drug (not a prodrug that needs switching on), lower levels could mean it works less well against the tumor, which is the last thing anyone wants.

I want to be honest about the evidence though: this is only test-tube and animal data, and it has never actually been seen in people. So it's a theoretical concern, not a proven one. Still, because larotrectinib is a serious cancer treatment, please loop in your oncologist or pharmacist before adding lentinan rather than deciding on your own.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
