Why this applies to Alogliptin

Alogliptin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the good news for anyone taking alogliptin (Nesina): this interaction is unlikely to be a real problem. The concern on file is that lentinan, a shiitake mushroom extract, might speed up a liver enzyme called CYP3A4. If a drug leans on that enzyme to be cleared, faster clearance could mean lower drug levels and weaker effect.

But alogliptin barely uses CYP3A4. It's mostly removed unchanged by your kidneys, so this pathway isn't really how your body handles it. On top of that, this is lab-only research that hasn't been seen in actual people. I wouldn't expect your blood sugar control to change. Still, keep up your normal glucose checks, and run any new supplement past your pharmacist or doctor first.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
