Why this applies to Mifepristone

Mifepristone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Korlym (mifepristone), the version used to manage high blood sugar in certain Cushing's syndrome. Your liver clears mifepristone using an enzyme called CYP3A4. Lab and animal studies hint that lentinan might rev up that enzyme, which could break down mifepristone a little faster and lower the amount in your body. In plain terms, that might make it work less well, so your blood sugar and cortisol-related symptoms could be harder to control.

That said, I want to keep this in perspective. This has only been seen in test-tube and animal research, never reported in people, so it's more of a theoretical heads-up than a proven problem. Still, before adding lentinan, please run it by your doctor or pharmacist so they can keep an eye on things.

What Is Mifepristone (Korlym) Used For?

Brand names: Mifeprex, Korlym

Korlym is prescribed for adults with endogenous Cushing's syndrome — a condition where the body produces too much cortisol — who also have type 2 diabetes or trouble controlling blood sugar. It works to bring those blood sugar levels under control.

It is only for blood sugar problems that are directly caused by Cushing's syndrome. It should not be used to treat ordinary type 2 diabetes unrelated to this condition.
