Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the piece of this combo pill that matters most for lentinan: the caffeine. Caffeine is cleared by an enzyme called CYP1A2, and lab and animal studies hint that lentinan might speed that enzyme up. If that happened in people, your body could clear caffeine a bit faster, so the small caffeine boost in Fiorinal with Codeine might feel weaker. This is theoretical and has never actually been reported in humans, so I wouldn't lose sleep over it.

The aspirin, butalbital, and codeine parts aren't really driven by CYP1A2, so they shouldn't change much here. Still, since lentinan products vary a lot and codeine is a strong medicine, it's smart to check with your pharmacist or doctor before pairing the two.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with Fiorinal with Codeine: the codeine part is a prodrug. It doesn't do much until your liver's CYP2D6 enzyme converts it into morphine, which is what actually relieves pain. Lab studies hint that lentinan might speed up CYP2D6. If that happened in real life, your body could turn codeine into morphine a little faster, which in theory could mean slightly stronger pain relief but also more morphine-type side effects like drowsiness, constipation, or nausea.

The honest part: this is only test-tube and animal data, and it has never been seen in people. So I wouldn't lose sleep over it. The aspirin, butalbital, and caffeine pieces aren't really affected. Still, run it by your pharmacist or doctor before adding lentinan, just to be safe.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Fiorinal with Codeine. Lab and animal studies hint that lentinan might speed up the CYP3A4 enzyme, which is one of the pathways that handles the butalbital piece of this combo. If that enzyme runs faster, butalbital could clear a bit quicker, which might mean less of the relaxing, headache-easing effect you're after.

Honestly though, this has never been shown in real people, so I wouldn't lose sleep over it. The codeine part actually depends on a different enzyme (CYP2D6) to switch into its active form, so this particular interaction doesn't really touch the pain relief from codeine. Just keep an eye on whether your medicine seems to work as well, and check with your pharmacist or doctor before adding lentinan.
