Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

The piece of this combo pill that matters here is the aspirin. Aspirin already thins the blood by making your platelets less sticky, which is part of why it can cause bruising, stomach upset, or bleeding. Danshen seems to nudge clotting in the same direction, so stacking the two could add up and raise your bleeding risk more than aspirin alone.

To be fair, this concern is mostly theoretical and based on animal studies and scattered case reports, not solid human trials. Still, since the effects point the same way, I'd play it safe. Watch for easy bruising, nosebleeds, bleeding gums, or dark stools, and check with your pharmacist or doctor before pairing Danshen with this medication.

In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's what stands out with Fiorinal with Codeine: this combo pill includes caffeine, which your body clears using a liver enzyme called CYP1A2. In test-tube studies, Danshen seemed to slow that enzyme down. If that happened in real life, caffeine could hang around longer, so you might feel more jittery, have trouble sleeping, a racing heartbeat, or an upset stomach.

The honest part: this is theoretical. When researchers tested Danshen with another CYP1A2 drug in actual people, they didn't see a real effect, so any change is likely small. The bigger thing to watch is that both Danshen and the aspirin in this pill can thin your blood, which may raise bruising or bleeding risk. Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This combination medicine, Fiorinal with Codeine, has a few moving parts. The one that interacts here is butalbital, the barbiturate piece that's handled by the liver and gut enzyme CYP3A4. The research on Danshen is a little tricky: taken regularly for a week or two, it seems to speed that enzyme up, which could lower butalbital levels and make it work less well. But a single dose of Danshen did the opposite and pushed levels up.

So depending on how you use it, you might notice either weaker headache relief or extra drowsiness and grogginess from the butalbital. The codeine part is a prodrug handled by a different enzyme, so this particular interaction is less about that. This is moderate, possible, and based on limited human data, so please check with your pharmacist or doctor before combining them.

Theoretically, danshen may increase the levels of aspirin and the risk of bleeding.
Research in healthy adult males shows that taking a combination of danshen and kudzu with aspirin increases plasma aspirin area under the curve by approximately 3.4-fold. Animal research also shows that taking a combination of danshen and kudzu (danshen-gegen formula) with aspirin increases maximal blood levels of aspirin and salicylic acid by approximately 4-fold and 3.7-fold, respectively, without impacting blood loss. Taking danshen increases the antiplatelet activity of aspirin and might increase the side effects of aspirin.

This combination pill contains aspirin, and that's the piece Danshen interacts with. In a small study of healthy men, pairing Danshen (combined with kudzu) with aspirin pushed aspirin blood levels up several-fold. On top of that, Danshen has its own blood-thinning, anti-clotting effect. Stacked together, that can add up to a higher chance of bleeding and bruising.

For you, that might look like easy bruising, nosebleeds, bleeding gums, or stomach irritation and ulcers (a known aspirin risk). The evidence here is early, but it points in a clear direction, so it's worth respecting. Please don't change anything on your own. Check with your pharmacist or doctor before combining Danshen with this medicine, especially if you also take other blood thinners.
