Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the situation with these two. Aliskiren is partly handled by a liver enzyme called CYP3A4. Lab and animal studies hint that lentinan might speed that enzyme up a bit. If that happened in people, your body could clear aliskiren a little faster, which in theory might soften its blood-pressure-lowering effect. Aliskiren is not a prodrug, so faster clearance means less effect, not more.

The big caveat: this has only been seen in test-tube and animal research, never reported in actual patients, so I wouldn't lose sleep over it. Still, if you take both, it's reasonable to keep an eye on your blood pressure readings at home and let your pharmacist or doctor know before adding lentinan.

What Is Aliskiren Used For?

Brand name: Tekturna

Aliskiren tablets — available as the brand Tekturna and as generic aliskiren — are used to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering blood pressure reduces the risk of strokes and heart attacks.

Controlling blood pressure is most effective as part of a broader plan that may include healthy eating, exercise, reducing sodium, and managing other risk factors like cholesterol and blood sugar. Many people need more than one blood pressure medicine to reach their goal.
