Why this applies to Darolutamide

Darolutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know about Nubeqa (darolutamide): your body relies on an enzyme called CYP3A4 to help process it. Danshen can nudge that enzyme, but the direction depends on how you take it. Taken steadily for a week or two, Danshen seems to speed up CYP3A4, which could lower your darolutamide levels and, in theory, make it a little less effective at holding your prostate cancer in check. A single dose appeared to do the opposite and raise levels.

Darolutamide is an active drug, not a prodrug, so more of it means stronger effects (and possibly more fatigue or aches), while less means weaker cancer control. The evidence here is early and modest, but because this medicine matters for your treatment, please loop in your oncologist or pharmacist before combining them.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the connection worth knowing: your prostate cancer medicine, darolutamide (Nubeqa), is moved around and partly cleared from your body by a transporter pump called P-glycoprotein. Danshen appears to slow that pump down. In one study in healthy people, danshen raised blood levels of another P-glycoprotein drug by around 30 to 40 percent.

If the same thing happens with darolutamide, more of it could build up in your system, which may mean a bit more fatigue, pain in your arms or legs, or rash. This is an active drug (not a prodrug), so the worry is too much, not too little. The evidence here is moderate and not certain, so please don't stop or change anything on your own. Just check with your oncologist or pharmacist before adding danshen.

About Darolutamide

What Is Darolutamide Used For?

Brand name: Nubeqa

Nubeqa (darolutamide) tablets are approved for adult men with two forms of advanced prostate cancer. The first is non-metastatic castration-resistant prostate cancer (nmCRPC) — cancer that keeps growing despite hormone-lowering therapy and has not yet spread to other parts of the body. The second is metastatic castration-sensitive prostate cancer (mCSPC) — cancer that has spread but still responds to hormone-lowering treatment.

For mCSPC, Nubeqa can be used alone or in combination with docetaxel (a chemotherapy medicine). In all cases, patients also receive a gonadotropin-releasing hormone (GnRH) agonist or antagonist — a medicine that lowers testosterone — or have already had surgery to remove the testicles.
