Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the piece that matters for your headache combo: the caffeine in it is cleared by a liver enzyme called CYP1A2. In test-tube studies, Danshen seems to slow that enzyme down. If that held true in people, caffeine could hang around a bit longer and feel stronger, meaning more jitteriness, a racing or pounding heart, trouble sleeping, or an upset stomach.

The honest part: this is only lab data. When researchers tested Danshen with a similar drug in actual humans, they didn't see a real change, so the concern stays mostly theoretical. Danshen can also affect blood clotting on its own, which is worth a separate mention. Before combining the two, check with your pharmacist or doctor so they can look at your full picture.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with your headache combo (Esgic, Fiogesic, and similar). The butalbital piece is partly cleared by a liver and gut enzyme called CYP3A4, and Danshen can nudge that enzyme in either direction. Taken for a stretch of about 10 to 14 days, Danshen tends to speed up that enzyme, which could lower butalbital levels and make this medicine work less well for your tension headaches. Oddly, a single dose of Danshen seems to do the opposite and may bump levels up.

Because butalbital is a barbiturate, higher levels can mean extra drowsiness, dizziness, or grogginess, while lower levels may just mean less relief. This is based on limited early research, so don't panic. Just run it by your pharmacist or doctor before combining them.

Here's the gist with Danshen and your headache medicine. One of the ingredients, acetaminophen (Tylenol), is partly broken down by a liver enzyme called CYP2E1. In lab and animal studies only, compounds in Danshen seem to slow that enzyme down. In theory, that could let acetaminophen build up a bit, and the main worry with too much acetaminophen over time is stress on the liver.

I want to be honest with you, though: this has never actually been reported in people, so it stays in the theoretical bucket. The bigger, better-documented concern is that Danshen can thin the blood. If you regularly take this combo, just give your pharmacist or doctor a heads-up before adding Danshen, and don't exceed the acetaminophen on the label.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the situation with this headache combo and Danshen. The acetaminophen part of your Tecnal or Esgic gets cleared partly by a process called glucuronidation, which is basically how your liver tags the drug so your body can remove it. In lab and animal studies, Danshen seems to tug on those same liver enzymes in both directions, sometimes speeding them up and sometimes slowing them down. So in theory it could shift how much acetaminophen lingers in your system.

The honest truth is this has never actually been reported in people, and the evidence is only test-tube and animal data, so I wouldn't lose sleep over it. Still, since acetaminophen is hard on the liver in higher amounts, it's worth a quick chat with your pharmacist or doctor before pairing them, especially with concentrated Danshen supplements.

What Is Acetaminophen, Butalbital, and Caffeine Used For?

Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).

All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
