Why this applies to Bupivacaine, Meloxicam

Bupivacaine, Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

The Zynrelef kit pairs bupivacaine, a local numbing medicine, with meloxicam, an anti-inflammatory (NSAID). The meloxicam part is cleared by a liver enzyme called CYP2C9. In test-tube studies, compounds in danshen appear to slow that enzyme down. If that happened in real life, meloxicam could build up a bit and lead to stronger NSAID-type effects, like stomach upset, fluid retention, higher blood pressure, or a small bump in bleeding risk. Danshen itself may also thin the blood, which adds to that concern.

Here's the reassuring part: this is only theoretical, based on lab data, and has not been reported in people. Still, since Zynrelef is used around surgery, mention any danshen use to your surgeon, pharmacist, or doctor first.
