Why this applies to Atazanavir, Cobicistat

Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the concern with Evotaz (atazanavir plus cobicistat). Both pieces of this combo lean heavily on a liver and gut enzyme called CYP3A4. Atazanavir is cleared by it, and cobicistat is actually there on purpose to block that enzyme and keep atazanavir levels high. Danshen can interfere with this same system. With regular use over a week or two, early human research suggests Danshen speeds up CYP3A4 in the gut, which could lower atazanavir levels. For an HIV medicine, weaker levels are a real worry because the virus needs steady drug to stay controlled.

To muddy things, a single dose of Danshen seems to do the opposite and raise levels. Either way, this is something to avoid guessing on. Please loop in your HIV provider or pharmacist before combining them.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
