Here's the gist for your Evotaz (atazanavir plus cobicistat): the cobicistat in it is a strong blocker of a liver enzyme called CYP3A4. That same enzyme helps clear modafinil, which is what your body makes when it converts adrafinil. So if you take adrafinil alongside Evotaz, the modafinil could hang around longer and build up, leaving you with more of its stimulant effects, things like trouble sleeping, a racing or pounding heart, headache, jitteriness, or feeling anxious.

I'll be honest, this is a theoretical concern based on how these drugs are processed, not on solid studies, so the real-world impact isn't proven. Still, adrafinil is unregulated and hard on the liver, so please don't add it without first checking with your doctor or pharmacist.

Here's the concern with pairing adrafinil and your HIV combo, Evotaz (atazanavir plus cobicistat). Once your liver turns adrafinil into modafinil, modafinil can mildly speed up an enzyme called CYP3A4. Atazanavir leans heavily on that enzyme, and cobicistat is added specifically to block it and keep your atazanavir levels high. So in theory, modafinil could nudge things in the opposite direction and lower your atazanavir levels.

Why that matters: with HIV medicine, levels that drop too low can let the virus rebound or build resistance, which you really want to avoid. To be clear, this is a theoretical interaction based on how these drugs behave, not strong proof. Still, please loop in your doctor or pharmacist before adding adrafinil, especially since it also stresses the liver.
