Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

This combination pill (Equagesic) contains aspirin, which already thins your blood by blocking platelets, the cells that help clots form. Danshen appears to do something similar, slowing clotting on its own. Stacking the two together could nudge your bleeding risk higher than aspirin alone.

In real life, that might show up as easier bruising, nosebleeds, bleeding gums, or stomach upset and dark stools (since aspirin can already irritate the stomach lining). To be fair, this concern is mostly theoretical, based on animal studies and scattered reports rather than strong human data. Still, since it's about bleeding, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you take other blood thinners.

Theoretically, danshen may increase the levels of aspirin and the risk of bleeding.
Research in healthy adult males shows that taking a combination of danshen and kudzu with aspirin increases plasma aspirin area under the curve by approximately 3.4-fold. Animal research also shows that taking a combination of danshen and kudzu (danshen-gegen formula) with aspirin increases maximal blood levels of aspirin and salicylic acid by approximately 4-fold and 3.7-fold, respectively, without impacting blood loss. Taking danshen increases the antiplatelet activity of aspirin and might increase the side effects of aspirin.

Equagesic combines aspirin with ethoheptazine and meprobamate, but it's the aspirin piece that matters here. Aspirin already thins the blood by blocking platelets from clumping. Danshen seems to push in the same direction, and one study in healthy men found that pairing danshen (with kudzu) alongside aspirin raised aspirin blood levels several-fold. More aspirin in your system can mean more of aspirin's usual problems: stomach upset, easy bruising, nosebleeds, or harder-to-stop bleeding.

The bigger concern is that danshen and aspirin both reduce clotting, so together the bleeding risk can stack up. This is concentrated supplement territory, not something you'd get from food. Please don't change anything on your own. Check in with your pharmacist or doctor before combining the two, especially if you ever notice unusual bruising or dark stools.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Equagesic combines three things: aspirin (a blood thinner and pain reliever), ethoheptazine (a mild pain reliever), and meprobamate (a sedative that calms anxiety and relaxes muscles). The concern here is that danshen may tinker with a liver clearance pathway called glucuronidation, which helps your body process some of these ingredients. In the test tube, danshen both sped up and slowed down these enzymes, so the net effect isn't clear cut.

If clearance slowed, levels could creep up, meaning more drowsiness or grogginess from the meprobamate, or a bit more bleeding or stomach upset risk from the aspirin. But this is only lab and animal data and has never been reported in real people. Danshen also independently affects clotting, which matters alongside aspirin. Please run this combination past your pharmacist or doctor before mixing them.
