Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the plain-English version. Danshen has been shown, mostly in test-tube and animal studies, to slow down a liver enzyme called CYP1A2. Some of the ingredients in this Vicks combo product lean on that pathway, so in theory danshen could let them build up a bit. If that happened, you might feel more of the drowsiness from chlorpheniramine (the antihistamine) or feel extra groggy or dizzy from dextromethorphan.
That said, the real-world evidence here is weak. When researchers tested danshen against a known CYP1A2 drug in actual people, they didn't see this slowdown happen. So this is more of a theoretical heads-up than a proven problem. The bigger thing I'd flag is that danshen may affect blood clotting on its own. Before combining them, it's worth a quick chat with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This cold and flu combo has three active ingredients, and the one most affected here is dextromethorphan (the cough suppressant), which is partly handled by the CYP3A4 pathway. Danshen has a tricky effect on this pathway. Taken regularly for a week or two, it tends to speed CYP3A4 up, which could lower dextromethorphan levels a bit and make it slightly less effective for your cough. But a single dose of danshen seems to do the opposite and can raise levels for a short time, which might mean more drowsiness or dizziness.
Honestly, this is short-term, occasional medicine, so the real-world impact is usually small. Still, since danshen may also affect blood clotting, it is worth a quick chat with your pharmacist before combining them.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.

Here's the practical angle for this combo cough-and-cold product. Of the three ingredients, the one tied to this interaction is acetaminophen, since a liver enzyme called CYP2E1 helps handle it. In lab tests, compounds in Danshen seem to slow that enzyme down. In theory, that could mean a bit more acetaminophen hangs around. The interesting twist is that CYP2E1 is also the pathway that turns some acetaminophen into a harmful byproduct, so slowing it could actually shift things, but honestly the real-world effect here is unclear.
The big takeaway: this is only test-tube evidence and has never been reported in real people, so I wouldn't lose sleep over it. The bigger acetaminophen rule still applies, don't double up products or exceed the daily limit. If you regularly use Danshen, just give your pharmacist a heads-up.

GLUCURONIDATED DRUGS

In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the deal with mixing Danshen and this multi-symptom cough and cold combo (Vicks Formula 44M). One way your body clears the acetaminophen in this product is through a process called glucuronidation. In lab and animal studies, Danshen has shown mixed effects on the enzymes that do this work, sometimes speeding them up and sometimes slowing them down. Because the signals point in both directions, it's genuinely hard to predict which way things would go in a real person.
The important thing to know: this is only theoretical, based on test-tube and animal research, and it has never actually been reported in people. So I wouldn't lose sleep over it.
