Ethinyl Estradiol, Levonorgestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Your birth control pill (the ethinyl estradiol and levonorgestrel combo) is partly handled by a liver enzyme called CYP1A2. In lab dish studies, Danshen seems to slow that enzyme down a bit. In theory, that could let a little more of the estrogen part linger in your body, which might nudge up estrogen-type side effects like nausea, breast tenderness, headaches, or bloating.

Here's the honest part: this comes only from test-tube research, and when scientists tried it in real people with a similar CYP1A2 drug, nothing meaningful happened. So I wouldn't lose sleep over it. Danshen also affects blood clotting, which deserves its own conversation since estrogen pills already carry a small clot risk. Please check with your pharmacist or doctor before combining them.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Your birth control pill (the ethinyl estradiol and levonorgestrel combo found in brands like Trivora and Triphasil) gets moved around the body partly by a transporter called P-glycoprotein. Danshen seems to slow that transporter down a bit. In a small study, it raised levels of another P-glycoprotein drug by around 30 to 40 percent, so it's possible Danshen could nudge your hormone levels up slightly too.

In real life this is a modest effect, and it isn't well studied with the pill specifically. If anything, higher hormone levels might mean a little more nausea, breast tenderness, or spotting. It's unlikely to make your birth control fail. Still, since Danshen can also thin the blood, I'd run it by your pharmacist or doctor before combining the two.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your birth control pill (ethinyl estradiol with levonorgestrel) is cleared from your body in part by a liver and gut enzyme called CYP3A4. The interesting wrinkle with Danshen is that it works two ways. Taken steadily for a week or two, Danshen appears to speed that enzyme up, which can clear the hormones out faster and lower their levels in your blood. With a combined pill, lower hormone levels can mean weaker protection against pregnancy and possibly more breakthrough bleeding or spotting.

This is based on early human research using a different drug, so it isn't proven for your specific pill, but I'd treat birth control failure as too important to gamble on. Please check with your pharmacist or doctor before pairing the two, and don't change anything on your own.
