Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the piece of this combo pill that actually connects to Danshen: the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and lab studies hint that Danshen might slow that enzyme down. If that happened in a real person, caffeine could linger longer than usual, so you might feel more jittery, have a racing heart, or trouble sleeping.

The honest part is that this is mostly theoretical. It came from test-tube research, and when scientists tested a similar CYP1A2 drug in actual people, they didn't see a real effect. So I wouldn't lose sleep over it. The codeine, acetaminophen, and salicylamide parts aren't the concern here. Still, since Danshen can also affect blood clotting, it's worth running this by your pharmacist or doctor first.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This combination pill is a mix, and the part to focus on with Danshen is the codeine. Codeine itself doesn't do much until your body converts it into morphine, so it's a prodrug. CYP3A4 plays a side role in handling codeine, and Danshen seems to nudge that enzyme. The catch is the timing: taking Danshen regularly for a week or two tends to speed up CYP3A4, while a single dose can briefly slow it down.

What does that mean for you? With steady Danshen use, your codeine could lose a little punch, so you might notice less pain relief. The other pieces (acetaminophen, caffeine, salicylamide) aren't strongly tied to this enzyme. The evidence here is modest, so don't panic, but please run it by your pharmacist or doctor before combining them.

This combo pill includes acetaminophen (Tylenol), and acetaminophen is partly handled by a liver enzyme called CYP2E1. In test tube studies, compounds in Danshen seem to slow down that enzyme. The idea on paper is that blocking CYP2E1 could shift how acetaminophen is processed, but here's the key point: this has only been seen in lab and animal work, never confirmed in real people. So it stays theoretical for now.

One thing worth a quick mention separately: Danshen may thin the blood a little, and this product contains salicylamide (aspirin-like). I wouldn't lose sleep over the CYP2E1 angle, but since several ingredients are in play here, it's smart to run this past your pharmacist or doctor before combining them.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

This combination pill has a lot going on, but the piece that touches Danshen is how your body clears acetaminophen (Tylenol). Acetaminophen is partly cleared by a process called glucuronidation, and lab studies suggest Danshen can tug on the enzymes that handle that step. The tricky part is that Danshen seems to both speed up and slow down those enzymes depending on how the root was prepared, so we honestly cannot predict which way it would go in a real person.

Right now this is only test-tube and animal data, and it has never been reported in actual patients. So I would not lose sleep over it, but if you take Danshen regularly, mention it to your pharmacist. The bigger, more practical concern with Danshen is its effect on bleeding and blood thinning, which is worth a separate conversation.
