Here's the part that matters with this combo: your medication contains caffeine, which is itself a stimulant. Ephedra (ma huang) is a much stronger stimulant on top of that. Stacking two stimulants together can push your heart rate and blood pressure higher, and may bring on a racing or pounding heartbeat, jitteriness, headache, trouble sleeping, or even chest discomfort. In rare cases this kind of combination has been tied to serious heart and blood pressure events.

This warning is based mainly on anecdotal reports rather than strong studies, so it's a theoretical concern. Still, ephedra is banned in U.S. supplements for good reason, and I wouldn't pair it with anything containing caffeine. Please check with your pharmacist or doctor before combining them.

The product you're asking about is a combination pain reliever, and one of its ingredients is caffeine, which is a stimulant in the same family this warning is about. Ephedra is also a powerful stimulant. When you stack two stimulants together, the effects can pile up on your heart and nervous system. That can mean a racing or pounding heartbeat, jitteriness, trouble sleeping, anxiety, and higher blood pressure.

This is a serious concern. There are reports linking ephedra plus caffeine to dangerous events like very high blood pressure, heart attack, stroke, seizures, and even death. Honestly, ephedra is banned in U.S. supplements for good reason, and I wouldn't recommend using it at all. Please talk with your pharmacist or doctor about safer options before combining anything like this.

Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

This combo product packs in caffeine, and that's the piece ephedra may bump up against. The thinking, based only on animal lab work, is that ephedra may speed up a liver enzyme called CYP1A2, which is the same enzyme that clears caffeine from your body. In theory that could mean caffeine leaves your system a bit faster, so the pick-me-up effect might feel weaker. The other ingredients (acetaminophen, codeine, salicylamide) aren't really driven by this enzyme, so they aren't the concern here.

Honestly, the much bigger worry is stacking two stimulants. Both ephedra and caffeine rev up your heart and blood pressure, and that combination can leave you jittery, racing, or with a pounding heart. Ephedra is banned in U.S. supplements for good reason. Please check with your pharmacist or doctor before mixing these.

This combo pill packs acetaminophen (Tylenol), caffeine, codeine, and salicylamide. The main liver concern here is the acetaminophen piece, which is hard on the liver if you take too much or stack it with other things that stress the liver. Ephedra has been tied to a number of real liver injury cases of its own. Putting the two together could, in theory, add up to extra strain on your liver.

I want to be honest that this is a theoretical worry based on how these things behave, not a long list of proven cases for this exact pairing. Still, ephedra is banned in supplements in the U.S. for good reason, and I would not recommend using it at all. Please check with your pharmacist or doctor before mixing anything new with this medication.
