Why this applies to Gilteritinib Fumarate

Gilteritinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with your leukemia medication, Xospata (gilteritinib). Your body relies on an enzyme called CYP3A4 to handle this drug, and Danshen can nudge that enzyme in either direction. When taken regularly for a week or two, Danshen seems to speed up CYP3A4, which could clear gilteritinib out faster and possibly weaken how well it fights the cancer. Oddly, a single dose did the opposite in studies, raising drug levels.

Because gilteritinib is doing such important work, I wouldn't want anything throwing off its levels. Higher levels could mean more side effects (tiredness, nausea, swelling, or heart rhythm changes), while lower levels could mean less benefit. Please don't add Danshen without first checking with your oncologist or pharmacist.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Xospata (gilteritinib): your body relies on a transporter called P-glycoprotein to help move this drug along and clear it. Danshen appears to slow that transporter down. In one small study in healthy volunteers, danshen raised levels of another P-gp drug by roughly 30 to 40 percent. If the same thing happens with gilteritinib, more of it could linger in your system, which may mean stronger side effects like fatigue, muscle or joint pain, swelling, low blood pressure, or changes in your heart rhythm (QT prolongation).

Because gilteritinib treats a serious leukemia and already needs careful monitoring, I'd play it safe. Please don't start or stop anything on your own. Talk with your oncology team or pharmacist before adding Danshen so they can weigh it for you.

What Is Gilteritinib Used For?

Brand name: Xospata

Xospata (gilteritinib) is used to treat adults with acute myeloid leukemia (AML) — a fast-moving blood and bone marrow cancer — when the disease has come back after prior treatment (relapsed) or stopped responding to treatment (refractory). It is only used when a laboratory test approved by the FDA has confirmed that the leukemia cells carry a FLT3 mutation, a genetic change that helps the cancer grow.

Because response can take time, treatment for at least 6 months is generally recommended as long as the disease is not progressing and side effects are manageable.
