Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the situation with combining Danshen and itraconazole. Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs around and help clear them. In one small study, Danshen slowed this pump down, which raised the levels of another medicine that relies on it. Itraconazole is also handled by P-glycoprotein, so it's possible Danshen could let a bit more of it build up in your blood.

If that happened, you might notice more of itraconazole's usual side effects, like nausea, stomach upset, headache, or swelling in your legs. Because itraconazole can also stress the heart and liver, I'd play it safe. The evidence here is limited and not strong, so don't panic, but please check with your pharmacist or doctor before pairing these two.

Itraconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist: your body uses an enzyme called CYP3A4 to clear itraconazole (Sporanox, Tolsura), and Danshen can nudge that enzyme. When taken steadily for a week or two, Danshen seems to speed up CYP3A4 in the gut, which could lower how much itraconazole gets into your blood. For an antifungal, weaker drug levels matter because the medicine may not fully knock out a stubborn nail, lung, or throat infection.

There's a twist worth knowing: a single dose of Danshen appeared to do the opposite and raise levels, which could mean more nausea, stomach upset, or liver strain. The human evidence is still early and modest, so don't panic. Just loop in your pharmacist or doctor before pairing these two.

About Itraconazole

What Is Itraconazole Used For?

Brand names: Sporanox, Sporanox Injection, Tolsura

Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.

Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
