CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Itraconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the thing with grapefruit and itraconazole: grapefruit blocks an enzyme in your gut and liver called CYP3A4, and that same enzyme helps clear itraconazole from your body. When it's slowed down, more itraconazole stays in your bloodstream, so levels can climb higher than intended. Itraconazole is an active drug (not a prodrug), so more drug means stronger effects and a bigger chance of side effects like nausea, stomach upset, headache, or liver strain. At high levels it can also affect the heart's rhythm.
One important note: this enzyme-blocking effect can linger for up to 48 hours, so spacing them apart doesn't really fix it. I'd skip regular grapefruit or grapefruit juice while you're on itraconazole, and check with your pharmacist or doctor first.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with grapefruit and itraconazole. Itraconazole isn't a prodrug, so we're really talking about how much of it your body soaks up. Grapefruit can mess with that absorption, but here's the tricky part: the research shows it can go either way. In some studies grapefruit juice raised itraconazole levels, and in others it actually lowered them. So the result is hard to predict for any one person.
Why does that matter? Too little drug and your fungal infection (nails, lungs, throat) may not clear up. Too much and you're more likely to get nausea, headache, or stomach upset. The evidence here is only moderate quality, so don't panic, but it's worth a quick chat with your pharmacist or doctor before adding grapefruit regularly.

P-GLYCOPROTEIN SUBSTRATES

While most in vitro research shows that grapefruit products inhibit P-gp,, research in humans is less clear. Two small clinical studies in healthy adults using digoxin as a probe substrate show that grapefruit juice does not inhibit P-gp in the kidneys. It is unclear whether this applies to intestinal P-gp, for which digoxin is not considered to be a sensitive probe. Grapefruit juice has been shown to reduce levels of fexofenadine, and increase levels of quinidine. However, as both of these drugs are also substrates of other enzymes and transporters, it is unclear what role, if any, intestinal P-gp has in these findings.

Here's the honest picture with grapefruit and itraconazole. The interaction we have on file is about grapefruit and P-glycoprotein, a transporter that helps move drugs around. That part is rated minor, and the human evidence for it is weak and a bit conflicting, so by itself it isn't something to lose sleep over.
That said, itraconazole is also handled by the CYP3A4 enzyme system in your gut, and grapefruit is well known for slowing that pathway. In theory that could let a little more itraconazole get into your body, which might mean more of its usual effects, like nausea, stomach upset, headache, or in rare cases liver strain. The size of this effect is uncertain. An occasional bite of grapefruit is unlikely to matter much, but regular grapefruit juice is worth running past your pharmacist or doctor first.

About Itraconazole
What Is Itraconazole Used For?
Brand names: Sporanox, Sporanox Injection, Tolsura
Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.
Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
