Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.

Here's the simple version: Saccharomyces boulardii is a live, beneficial yeast, and itraconazole (Sporanox, Tolsura) is an antifungal whose whole job is to slow down and kill yeasts and fungi. So if you take them at the same time, the medicine may end up knocking out some of the very probiotic you're trying to take. This doesn't make the itraconazole dangerous or weaker for your infection; it just means the probiotic may not work as well.

Good news: this is a theoretical, minor concern based on how the two work, not strong patient studies. If you and your doctor decide you want both, your pharmacist can help you space them out so the probiotic has a better chance to survive. Always check before starting anything new.

Saccharomyces boulardii is a beneficial yeast used as a probiotic, with the strongest evidence for preventing and treating certain types of diarrhea, including antibiotic-associated diarrhea. It is generally well tolerated in healthy people, but those who are seriously ill or have weakened immune systems should avoid it because of rare but serious bloodstream infections. About Itraconazole

What Is Itraconazole Used For?

Brand names: Sporanox, Sporanox Injection, Tolsura

Itraconazole capsules treat several serious fungal infections: blastomycosis, histoplasmosis, and aspergillosis (in people who cannot tolerate or have not responded to amphotericin B). Sporanox and generic itraconazole capsules are also used for toenail and fingernail fungus (onychomycosis) in people with normal immune systems.

Itraconazole oral solution has a different use: it treats oropharyngeal and esophageal candidiasis (yeast infection, or thrush, of the mouth, throat and esophagus). Tolsura capsules are approved only for the three serious infections in adults, not for nail fungus.
