Here is something a lot of folks don't realize: Saccharomyces boulardii is actually a live yeast, and miconazole is an antifungal whose whole job is to kill yeast and fungus. So the concern here flows in one direction. Taking them together could knock down some of the probiotic, making it less effective. It does not make miconazole stronger or more dangerous, and it won't increase side effects like the itching, burning, or skin irritation people sometimes get from the cream or vaginal products.

Keep in mind this is a theoretical interaction based on how the two work, not something firmly proven. With topical or vaginal miconazole, very little reaches the gut anyway, so the real-world impact is probably small. Still, if you're taking S. boulardii for diarrhea, it's worth a quick chat with your pharmacist about timing.
