Here's the simple version: Saccharomyces boulardii is a live, beneficial yeast, and voriconazole (Vfend) is an antifungal whose whole job is to slow the growth of yeasts and fungi. So if you take them at the same time, the worry isn't that your voriconazole becomes dangerous. It's the opposite direction: the antifungal may kill off a good chunk of the probiotic yeast, leaving you with less of the benefit you were hoping for, like protection against diarrhea.

I want to be honest that this is a theoretical concern based on how these two work, not something proven in studies, and it's rated minor. Your voriconazole should still treat your fungal infection as expected. Since voriconazole is a serious medicine often used in people who are quite ill, please run any probiotic past your doctor or pharmacist first.

Saccharomyces boulardii is a beneficial yeast used as a probiotic, with the strongest evidence for preventing and treating certain types of diarrhea, including antibiotic-associated diarrhea. It is generally well tolerated in healthy people, but those who are seriously ill or have weakened immune systems should avoid it because of rare but serious bloodstream infections. About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
