Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Duavee (bazedoxifene with conjugated estrogens). One of its ingredients, bazedoxifene, is handled in part by a transporter pump in your gut called p-glycoprotein. Danshen appears to slow that pump down. In a study with another drug that uses the same pump, danshen raised blood levels by roughly a third. So in theory, danshen could let a bit more of your medication build up in your body.

If that happens, you might notice more of the usual estrogen-type effects, like breast tenderness, bloating, headaches, or nausea. The evidence here is limited and based on a different drug, so this is more a heads-up than a red alert. Don't stop or change anything on your own, just check with your pharmacist or doctor before adding danshen.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

If you're taking Duavee (which combines conjugated estrogens with bazedoxifene) for hot flashes or bone protection, here's the thing about Danshen. Both of these ingredients are partly broken down by a liver and gut enzyme called CYP3A4. When Danshen is taken steadily for a week or two, it seems to speed up that enzyme, which could push more of your medicine out of your body faster. In plain terms, your Duavee might not work quite as well, so you may notice hot flashes creeping back or less benefit for your bones over time.

One odd wrinkle: a single dose of Danshen appears to do the opposite and briefly raise drug levels. The evidence here is limited and comes from a small study using a different drug, so we're really estimating. Please don't stop or change anything on your own. Just loop in your pharmacist or doctor before adding Danshen.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the short version on combining Danshen with Duavee (conjugated estrogens plus bazedoxifene). Both the estrogen part and bazedoxifene are cleared from your body mainly through a process called glucuronidation, where the liver tags them so they can be flushed out. In lab and animal studies only, Danshen seems to nudge these glucuronidation enzymes in both directions, sometimes speeding them up and sometimes slowing them down, partly depending on how the root was processed.

Because the direction isn't clear and this has never been reported in actual people, I wouldn't lose sleep over it. In theory it could shift your hormone levels a bit, which might mean more breakthrough hot flashes or, less likely, more estrogen-type side effects. Just mention the Danshen to your pharmacist or doctor before starting it.

What Is Estrogen and Bazedoxifene Used For?

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
