Loncastuximab Tesirine-Lpyl falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the idea behind this pairing. Danshen seems to slow down a cellular pump called p-glycoprotein, which helps move certain drugs out of your body. In a small study, danshen raised blood levels of a p-glycoprotein drug by about 30 to 40 percent. Since Zynlonta (loncastuximab tesirine) is listed as a p-glycoprotein substrate, the theory is that danshen could let a bit more of it linger in your system.

With a strong chemo-type cancer medicine like this one, even modest extra exposure could mean more side effects, things like low blood counts, fatigue, swelling, or liver enzyme changes. The evidence here is limited and indirect, so I wouldn't panic, but I also wouldn't combine them on your own. Please run it by your oncology team or pharmacist first.

  • Loncastuximab tesirine-lpyl injection is used to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not respond to or has returned after treatment with at least two other cancer treatments. Loncastuximab tesirine-lpyl is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
