Erythromycin falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist: erythromycin gets shuttled around your body partly by a transporter called P-glycoprotein, which acts like a pump that escorts the drug back out of cells and into your gut and urine. In lab (test-tube) experiments, Rabdosia rubescens seems to slow that pump down. If that happened in people, a bit more erythromycin could stick around, which might nudge up its usual side effects like stomach upset, nausea, cramping, or diarrhea.

I want to be straight with you, though: this is based only on laboratory data, not real human studies, so we don't truly know if it matters when you swallow a pill. Since erythromycin can also affect heart rhythm at higher levels, I'd play it safe and check with your pharmacist or doctor before pairing the two rather than guessing.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Erythromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for erythromycin. Your body clears this antibiotic in part through a liver enzyme called CYP3A4. In test-tube and animal studies, a compound in Rabdosia rubescens called oridonin seems to speed up that enzyme. If that happens in people, your body could break down erythromycin faster, leaving lower levels in your blood. Since erythromycin works by reaching high enough levels to stop bacteria from growing, lower levels could mean it doesn't fully knock out your infection.

I want to be honest about the strength of this: it's only lab and animal data, and it has never been confirmed in humans. So this is a theoretical concern, not a proven one. Still, since you're trying to clear an infection, it's worth a quick chat with your pharmacist or doctor before combining the two.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
