Prednisolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist: prednisolone is one of the medicines your body partly moves around using a transporter called p-glycoprotein. Lab studies suggest Rabdosia rubescens may slow that transporter down. In theory, that could let a bit more prednisolone hang around in your body, nudging its effects (and side effects) up. With steroids, that might look like more fluid retention, higher blood sugar, trouble sleeping, mood changes, or stomach upset.

I want to be honest about how soft this evidence is. It comes only from test-tube research, not from people, so we don't really know if it matters in everyday life. Prednisolone is not a prodrug, so the direction here is simply more, not less. Still, before combining the two, please run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Prednisolone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for Prednisolone. Your liver uses an enzyme called CYP3A4 to help break this steroid down. Lab and animal studies hint that a compound in Rabdosia rubescens, called oridonin, may speed that enzyme up. If that happens in people, your body could clear prednisolone a little faster, which might leave you with less steroid effect than expected.

Why that matters: if you're taking prednisolone for something like asthma, arthritis flares, an autoimmune condition, or to prevent transplant rejection, a weaker effect could mean returning symptoms or flares. Honestly though, this is only test-tube and animal evidence, not confirmed in humans, so I wouldn't panic. Just loop in your pharmacist or doctor before adding this herb, especially since steroid doses are often carefully balanced.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.
