Why this applies to Ciclesonide

Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's how this one shakes out for Alvesco (ciclesonide). This inhaler delivers a corticosteroid that your liver and lung tissue activate and then break down using an enzyme called CYP3A4. Lab research suggests oridonin, a compound in Rabdosia rubescens, may speed that enzyme up. If that held true in people, your body could clear the active steroid a bit faster, which in theory might make your asthma controller work less well over time (more wheezing, coughing, or chest tightness creeping back).

I want to be straight with you, though: this is only test-tube and animal data, and it has never been reported in actual patients. So it's worth a quick mention to your pharmacist or doctor before combining them, but not a reason to panic.

What Is Ciclesonide Oral Inhalation Used For?

Brand names: Omnaris, Alvesco, Zetonna

Alvesco (ciclesonide) inhaler is approved for the maintenance treatment of asthma as a preventive therapy in adults and children 12 years of age and older. It is taken regularly to reduce airway inflammation and help prevent asthma symptoms — not to relieve a sudden attack.

Alvesco is not a rescue inhaler and should not be used to treat sudden wheezing or acute asthma episodes. It is also not approved for children under 12.
