Why this applies to Lonafarnib

Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist: Danshen may slow down one of the liver enzymes (CYP2C9) that helps clear lonafarnib from your body. When that enzyme is partly blocked, lonafarnib can build up to higher levels than expected. Since lonafarnib is an active drug (not a prodrug that needs switching on), higher levels usually mean a stronger effect and more side effects, things like nausea, vomiting, diarrhea, poor appetite, tiredness, or changes in lab results your care team watches.

I want to be honest about the strength of this: it's theoretical, based on lab studies only, and hasn't been reported in real patients. Still, lonafarnib is a specialized medicine, so don't start or stop Danshen on your own. Please check with your doctor or pharmacist first.

Lonafarnib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the short version for Zokinvy (lonafarnib): your body clears this drug using a liver and gut enzyme called CYP3A4, and Danshen can mess with that same enzyme. The catch is that Danshen seems to work in two directions. Taken regularly for a couple of weeks, it appears to speed up CYP3A4, which could lower your lonafarnib levels and make it less effective. But a single dose seems to do the opposite and push levels up, which could mean more side effects like nausea, vomiting, diarrhea, poor appetite, or fatigue.

Because lonafarnib is a precise, important medicine, even modest swings matter. Please don't add or stop Danshen on your own. Run it by your pharmacist or doctor first.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
