Bexarotene falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to know about bexarotene (Targretin): your body relies on a liver and gut enzyme called CYP3A4 to break it down. The research on Danshen is a little tricky, because taking it for a stretch of time (about 10 to 14 days) seems to speed up that enzyme, while a single dose may briefly slow it down.

If Danshen revs up CYP3A4 the way the studies suggest, your body could clear bexarotene faster and leave you with less of it, which may mean the cancer medicine doesn't work as well as it should. That's a real concern for a drug treating a serious condition like cutaneous T-cell lymphoma. The evidence here isn't rock-solid, so please don't change anything on your own. Just loop in your oncologist or pharmacist before adding Danshen.

What Is Bexarotene Used For?

Brand name: Targretin

Bexarotene capsules (brands: Bexarotene, Targretin) are used to treat the skin symptoms of cutaneous T-cell lymphoma (CTCL) in patients whose disease has not responded to at least one prior systemic treatment.

Bexarotene gel 1% (brands: Bexarotene, Targretin) is used to treat the skin lesions of early-stage CTCL (Stage IA and IB) when other therapies have not worked or could not be tolerated. The gel is a topical therapy and is not intended for systemic use.
