Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Danshen and your Sirturo (bedaquiline). Bedaquiline relies on an enzyme called CYP3A4 to be broken down. The early research we have suggests that taking Danshen for about 10 to 14 days actually speeds that enzyme up, which means your body could clear bedaquiline faster than intended. In plain terms, you might end up with lower drug levels, and that's a real concern with TB treatment, where keeping steady, effective levels is what helps stamp out a stubborn, drug-resistant infection.

This isn't an active drug being supercharged, it's the opposite worry: losing potency over time. The evidence is preliminary and was studied with a different medicine, so we can't be certain. Please don't stop or change anything on your own. Just loop in your TB doctor or pharmacist before adding Danshen.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
