Bosentan falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the simple version: Bosentan works to open up the blood vessels in your lungs, and it can lower blood pressure as part of how it works. Danshen, in animal studies, seems to lower blood pressure too. So the worry is that stacking the two together could push your blood pressure a little too low.

If that happened, you might feel dizzy, lightheaded, tired, or even faint, especially when standing up quickly. I'll be honest with you though: this concern comes from lab and animal research, not from solid human studies, so it's more of a theoretical caution than a proven problem. Danshen may also affect blood clotting, which is its own separate issue. Before combining them, please check with your pharmacist or doctor so they can keep an eye on your blood pressure.

Bosentan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist for bosentan (Tracleer). Your body relies on a liver enzyme called CYP2C9 to help clear bosentan. Danshen, in lab studies, appears to slow that enzyme down. If it slows the cleanup, more bosentan could linger in your system, which in theory could mean stronger effects and more side effects like headache, flushing, swelling in your legs or ankles, low blood pressure, or extra strain on your liver.

I want to be honest about the evidence: this is only seen in test-tube and animal research, not in real people, so it's a theoretical concern. Still, since you take bosentan for a serious lung condition, please check with your pharmacist or doctor before adding Danshen rather than starting it on your own.

Bosentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist for Tracleer (bosentan). Your body relies on an enzyme called CYP3A4 to break it down. The research we have suggests that taking Danshen for a couple of weeks can speed that enzyme up, which means your body may clear bosentan faster than usual. If that happens, you could end up with lower drug levels and less benefit, so your pulmonary hypertension symptoms (shortness of breath, trouble exercising) might not be as well controlled.

One wrinkle: a single dose of Danshen seemed to do the opposite in studies, briefly raising levels. So the effect isn't fully predictable, and the evidence is still early. Since bosentan is a serious medicine for a serious condition, please don't add Danshen without first checking with your pharmacist or doctor.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
