Irbesartan, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Your CoAprovel combines two blood pressure lowerers: irbesartan (which relaxes blood vessels) and hydrochlorothiazide (a water pill). Danshen, on its own, has shown blood pressure lowering effects in animal studies. So the concern here is simple addition: putting Danshen on top of a medicine that already brings your pressure down could push it a little too low.

If that happened, you might feel dizzy, lightheaded, tired, or woozy when you stand up. I want to be honest though, this is a theoretical worry based mostly on lab and animal research, not strong human studies. It's worth a quick chat with your pharmacist or doctor before adding Danshen, and check your blood pressure if you do.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Irbesartan, Hydrochlorothiazide also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist with CoAprovel (irbesartan plus hydrochlorothiazide). The irbesartan part is partly cleared by a liver enzyme called CYP2C9. In test-tube studies, compounds in Danshen appear to slow that enzyme down. If that happened in the body, irbesartan could build up a bit, which might mean a stronger blood-pressure-lowering effect than you expect. Realistically that could show up as feeling lightheaded, dizzy, or tired, especially when standing up quickly.

I want to be honest about the strength of this, though. So far it's only been seen in the lab, not in real people, so it's more of a theoretical concern than a proven one. Danshen can also affect blood clotting on its own. Before combining them, please check in with your pharmacist or doctor.
