Brentuximab Vedotin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Adcetris (brentuximab vedotin): part of it, the toxic MMAE piece that actually fights the lymphoma, is broken down by an enzyme called CYP3A4. Danshen can mess with that same enzyme. The tricky part is the direction depends on how long you've taken it. Taken steadily for a week or two, Danshen seems to speed up CYP3A4, which could lower MMAE levels and might make treatment less effective. A single dose appears to do the opposite, raising levels and potentially worsening side effects like numb or tingling hands and feet, low blood counts, or nausea.

Honestly, the evidence here comes from a midazolam study, not from brentuximab itself, so it's somewhat indirect. Still, with a chemotherapy drug this potent, I wouldn't combine them without checking. Please run it by your oncologist or pharmacist first.

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
