Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Here's the gist: losartan (Cozaar) lowers your blood pressure, and Danshen may gently nudge it down too. In animal studies, Danshen widened blood vessels and lowered blood pressure on its own, and it added to the effect when paired with a related blood pressure drug. So stacking the two could push your numbers a little lower than you intend.

What might that feel like? Things like dizziness, lightheadedness, feeling tired, or a woozy spell when you stand up quickly. Keep in mind this warning is theoretical and based on animal data, not strong human studies, so I'm not ringing alarm bells. Danshen can also affect blood clotting, which is worth mentioning to your doctor. Before adding Danshen, please run it by your pharmacist or doctor so they can keep an eye on your blood pressure.

Losartan Potassium also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's a wrinkle worth knowing about losartan. Your body relies on an enzyme called CYP2C9 to turn losartan into its more active form, so losartan is partly a prodrug. Lab studies suggest Danshen can block that enzyme. In theory, blocking it could mean less of the active form is made, which might slightly soften losartan's blood-pressure-lowering punch. On the flip side, the parent drug could linger longer, so the net effect in real people is honestly hard to predict.

Here's my honest take: this is based only on test-tube research and has never been reported in actual patients, so I wouldn't lose sleep over it. Still, since blood pressure is involved, please loop in your pharmacist or doctor before adding Danshen, and keep an eye on your readings.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's something worth knowing about losartan (Cozaar): it's actually a prodrug. Your body uses a liver enzyme called CYP3A4 (along with another called 2C9) to turn losartan into its active form, which does most of the blood-pressure-lowering work. So the enzyme that this interaction touches is the same one that activates your medicine.

The research on Danshen here is mixed and fairly early. Taken steadily for a couple of weeks, Danshen seems to speed up CYP3A4, which could mean less of losartan's active form and possibly weaker blood pressure control. A single dose may briefly do the opposite. Either way, the effect on losartan isn't well proven, so don't panic. Just check with your pharmacist or doctor before adding Danshen, and keep an eye on your blood pressure readings.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
