Why this applies to Losartan Potassium

Losartan Potassium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice can increase levels of drugs metabolized by CYP3A4.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.

Here's a twist worth knowing about: losartan is actually a prodrug. Your body uses a liver enzyme (CYP3A4, along with CYP2C9) to convert losartan into its more powerful active form, called E-3174, which does most of the blood-pressure-lowering work. Grapefruit blocks that same CYP3A4 enzyme.

So with losartan, grapefruit can work in the opposite direction from many other drugs. By slowing the switch to the active form, grapefruit may make losartan a bit less effective, meaning your blood pressure could be less well controlled. The good news is the effect is usually modest, and an occasional bite of grapefruit isn't likely to cause trouble. Still, if you regularly drink grapefruit juice, mention it to your pharmacist or doctor so they can keep an eye on your readings.

Here's a twist worth knowing about losartan. Your body actually has to convert losartan into its more active form, and the enzyme CYP2C9 helps do that work. Grapefruit may slow that same enzyme down. So in theory, blocking it could mean less of the active drug forms, which might make losartan a little less effective at lowering your blood pressure.

The good news: this idea comes only from lab and test-tube research, and it has not actually been seen in real people taking losartan. So I wouldn't lose sleep over an occasional grapefruit. If you eat it often or notice your blood pressure creeping up, just mention it to your pharmacist or doctor before making any changes.

Losartan is an inactive prodrug which must be metabolized to its active form, E-3174, to be effective. In one human study, grapefruit juice reduced losartan metabolism, increased losartan AUC, and reduced the AUC of the major active losartan metabolite, E-3174.

Here's a wrinkle worth knowing about losartan: it's actually a prodrug. That means the pill you swallow isn't doing the heavy lifting on its own. Your liver has to convert it into an active form called E-3174, and that active version is what really lowers your blood pressure.

Grapefruit appears to slow down that conversion step. In one human study, grapefruit juice raised the level of plain losartan but lowered the amount of the active form your body actually uses. The practical worry is a bit less blood pressure control, so your numbers could creep up. The evidence here is moderate, not airtight, but it's reasonable to be cautious. Please don't change anything on your own. Just check with your pharmacist or doctor about whether regular grapefruit fits with your losartan.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
