Here's something worth knowing if you take losartan (Cozaar) for blood pressure. Losartan is actually a prodrug, meaning your liver has to convert it into its active form before it does much work. St. John's wort speeds up the liver enzyme (CYP3A4) involved in that process, and it also revs up the enzyme that clears the drug out. The net result reported in research is usually lower drug levels and weaker blood pressure control.

In real life, that could mean your blood pressure creeps back up, undoing some of the protection losartan gives you. This is a probable interaction backed by decent (though not perfect) evidence, so it's worth respecting. Please don't stop or change anything on your own. Just check in with your pharmacist or doctor before combining these two.

Here's the deal with pairing St. John's wort and your losartan (Cozaar): this isn't about your blood pressure control or how well the losartan works. Instead, both can make your skin a little more sensitive to sunlight. St. John's wort contains a compound called hypericin, which can cause your skin to sunburn or react more easily, and losartan falls into that same camp.

Honestly, this one is mostly theoretical, based on how these substances behave rather than strong real-world reports. Studies suggest you'd usually need very high doses of St. John's wort before skin problems show up. Still, if you're out in the sun a lot, simple steps like sunscreen and protective clothing help. Before adding St. John's wort, please check with your pharmacist or doctor, since this herb has many other serious interactions.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the wrinkle with losartan (Cozaar): it's actually a prodrug. Your liver uses an enzyme called CYP2C9 to turn losartan into its active form, which is what really lowers your blood pressure. St. John's wort can speed up that same enzyme, so in theory it could change how much active medicine you end up with.

Because this herb tends to rev up drug metabolism, the more likely real-world worry is that your losartan may not work as well, meaning your blood pressure could creep back up. Honestly, the research here is mixed and not strong, so this is more of a "keep an eye on it" situation than an emergency. If you want to try St. John's wort, please check with your pharmacist or doctor first and keep monitoring your blood pressure.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
