Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Danshen interacts with.

Theoretically, taking danshen with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that danshen can produce dose-dependent hypotensive effects. Furthermore, concomitant use with captopril appears to potentiate this effect.

Opsynvi combines two blood pressure lowering medicines: macitentan, which relaxes blood vessels in the lungs, and tadalafil, which also widens vessels. Both already nudge your blood pressure downward. Danshen may have its own blood pressure lowering action based on lab and animal studies, so stacking it on top could push your pressure lower than intended.

If that happens, you might feel dizzy, lightheaded, especially when standing up, or notice headache, flushing, or a faster heartbeat. Keep in mind this concern comes from early animal data, not solid human studies, so it is more of a caution than a proven problem. Still, please check with your pharmacist or doctor before adding Danshen, and mention if you feel faint.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Macitentan, Tadalafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Opsynvi combines two medicines, macitentan and tadalafil, and both partly rely on a liver enzyme called CYP2C9 to be cleared from your body. In test-tube studies, compounds in Danshen appear to slow that enzyme down. If that carried over into people, these drugs could linger and their effects could build up. In real life that might look like more low blood pressure, dizziness, headache, flushing, a stuffy nose, or swelling in your ankles.

Here is the important part: this is only theoretical, based on lab work, not actual patient reports, so I would not panic. Still, because Danshen can also thin the blood, it is worth a quick chat with your pharmacist or doctor before combining the two.

Macitentan, Tadalafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Opsynvi combines two medicines, macitentan and tadalafil, and both are broken down partly by an enzyme in your gut and liver called CYP3A4. Here's the tricky part about Danshen: taken regularly for a couple of weeks, it seems to speed that enzyme up, which could lower your levels of both drugs and make them work less well. A single dose, though, appears to do the opposite and raise levels. So the effect can swing in either direction.

If levels drop, your pulmonary hypertension may be less controlled. If they climb, you might notice more headache, flushing, low blood pressure, dizziness, or leg swelling. This is early evidence in healthy volunteers, not proof, but I'd play it safe. Please check with your pharmacist or doctor before combining them.
