Why this applies to Mometasone, formoterol

Mometasone, formoterol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Here's the honest picture for your Zenhale inhaler. The concern on file is that danshen may slow down a liver enzyme called CYP2C9, which could theoretically let certain medicines build up a bit. The good news is that mometasone and formoterol are mostly cleared by other pathways (CYP3A4 plays the bigger role here), and an inhaler delivers a small dose right to your lungs, so very little reaches your bloodstream to begin with.

If levels did creep up, you might notice more of formoterol's usual effects like a faster heartbeat, jitteriness, shakiness, or a headache. But remember, this is based only on lab studies, not real people. Danshen also affects blood clotting, which is its own reason to check in. Please run it by your pharmacist or doctor before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing about Zenhale (mometasone plus formoterol): you breathe it into your lungs, so most of the dose works right where you need it. Only a small amount reaches your bloodstream, where your liver and gut use an enzyme called CYP3A4 to clear it. Danshen can nudge that enzyme, but mostly for medicines you swallow, not ones you inhale.

Taken steadily for a couple of weeks, Danshen seems to speed CYP3A4 up, which could slightly lower the bit of mometasone that gets absorbed. One dose may briefly do the opposite. For an inhaler, this is unlikely to cause much real change. The evidence is early and based on a different drug. Still, since Danshen can also affect bleeding, it's worth a quick chat with your pharmacist before starting it.
