Hydrocodone, Ibuprofen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

The piece of your combo pill that matters here is the ibuprofen. Ibuprofen is an NSAID, and on its own it can already nudge up bleeding risk by making platelets a little less sticky and by irritating the stomach lining. Danshen has been reported to thin the blood and slow clotting too, so stacking the two could add up and raise your chance of bruising easily, nosebleeds, or stomach bleeding (think black or tarry stools or stomach pain).

Honestly, the evidence for the Danshen side is thin (mostly anecdotal and animal data), so I'm not trying to scare you. But because the bleeding concern is real with ibuprofen, I'd check with your pharmacist or doctor before pairing them, especially if you also take other blood thinners.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro research shows that various constituents of danshen inhibit the activity of CYP2C9. So far, this interaction has not been reported in humans.

Vicoprofen pairs hydrocodone (an opioid) with ibuprofen, and it's the ibuprofen half that matters here. Ibuprofen is partly broken down by a liver enzyme called CYP2C9. In test-tube studies, certain compounds in Danshen appear to slow that enzyme down. If that happened in your body, ibuprofen could clear a little slower and build up, which in theory might nudge up its usual side effects, like stomach upset, heartburn, a higher risk of stomach irritation or bleeding, or fluid retention.

I want to be honest about the strength of this: it's only lab data so far, and it has never actually been reported in people. So please don't panic. Still, since Danshen can also affect blood clotting on its own, it's worth a quick chat with your pharmacist or doctor before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Vicoprofen (hydrocodone plus ibuprofen): the hydrocodone part is partly handled by a liver and gut enzyme called CYP3A4. Danshen seems to nudge this enzyme in two directions. Taken steadily for a week or two, danshen speeds up that enzyme, which could clear hydrocodone faster and leave you with weaker pain relief. But a single dose of danshen does the opposite and may briefly raise levels, which could mean more drowsiness, dizziness, or slowed breathing.

Honestly, the evidence here is preliminary and based on a different drug, so the real-world effect on your pain medicine isn't certain. There's also a separate worry: danshen may thin the blood, and the ibuprofen part can already irritate the stomach and raise bleeding risk. Please check with your pharmacist or doctor before combining them.
