Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for Alvimopan (Entereg). Your body uses a kind of cellular pump called P-glycoprotein to move alvimopan around and clear it. Danshen appears to slow that pump down. We saw this in a small human study where danshen raised the blood levels of a similar pump-dependent drug by roughly a third. So taking danshen alongside alvimopan could let a bit more of the drug build up than expected.

The good news: alvimopan is a short-course, hospital-supervised medicine used right after bowel surgery, so this isn't something you'd usually be juggling on your own. The evidence here is moderate and the effect is possible, not certain. Still, please mention any danshen use to your surgical team or pharmacist before your procedure so they can keep an eye on things.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
