St. John's wort decreases the levels and clinical effects of P-glycoprotein substrates.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the gist for alvimopan (Entereg): it's carried in and out of your body by a transporter called P-glycoprotein, which acts like a little pump in your gut. St. John's wort speeds that pump up. The likely result is that less alvimopan gets absorbed, so its levels and its effect drop.

Since alvimopan is given short-term in the hospital to help your bowels wake up after surgery, weaker levels could mean it does its job less well, leaving you more prone to the slow gut, bloating, and delayed bowel movements it's meant to prevent. This is a real, probable interaction, though the evidence is moderate. Please don't start or stop anything on your own. Just let your surgical team, doctor, or pharmacist know about any St. John's wort before surgery.

What Is Alvimopan Used For?

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
