Darunavir, Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the short version: your HIV combination, darunavir with ritonavir, is partly handled by a cellular pump called P-glycoprotein. Danshen appears to slow that pump down a bit. In one small study, danshen raised blood levels of another P-glycoprotein drug by roughly a third. So in theory, taking danshen could nudge the levels of your medicine upward, which might mean more of the usual side effects like nausea, diarrhea, headache, or an upset stomach.

The evidence here is modest and comes from a lower-quality study using a different drug, so this is more of a caution than a certainty. Since staying steady on HIV therapy really matters, I wouldn't add danshen without first checking with your pharmacist or doctor, who can weigh it against your specific regimen.

Darunavir, Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the deal with adding Danshen to your darunavir/ritonavir combo. Both of these HIV medicines are handled by a liver and gut enzyme called CYP3A4, and Danshen can nudge that enzyme in either direction. In one small human study, taking Danshen daily for a week or two sped up this enzyme, which lowered blood levels of a test drug by a lot. If the same thing happens with your antivirals, you could end up with less drug in your system, and that risks weaker HIV control or resistance over time.

Oddly, a single dose of Danshen did the opposite and raised levels. That mixed picture, plus keeping your viral load suppressed, is exactly why I'd have you check with your pharmacist or HIV provider before combining these rather than deciding on your own.
