Acetaminophen, Dextromethorphan, Phenylpropanolamine, Pyrilamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

This combo product (Theracaps) is a multi-symptom cold and cough medicine. The piece that ties into Danshen is mainly how your liver's CYP1A2 enzyme helps process some of these ingredients. In test-tube studies, Danshen appeared to slow that enzyme down. If it did the same in people, the medicine could clear a little more slowly and its effects could feel stronger.

In real-world terms, that might mean more drowsiness or dizziness from the dextromethorphan, or feeling a bit jittery or having a faster heartbeat from the decongestant. Here's the reassuring part: this is a theoretical concern from lab work only. When researchers tested it in humans, Danshen did not meaningfully change a similar drug. Still, if you take Danshen regularly, it's worth a quick chat with your pharmacist before combining them.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

This combination cold and cough product (Theracaps) contains a few ingredients, and the one most affected here is dextromethorphan, the cough suppressant. Your body partly clears it through an enzyme called CYP3A4. The research on Danshen is a little tricky: taken steadily for a couple of weeks, it seems to speed up that enzyme, which could lower dextromethorphan levels and make the cough relief feel weaker. But a single dose of Danshen may do the opposite and raise levels for a short time.

In plain terms, the effect is unpredictable and likely small for most people. If you notice extra drowsiness, dizziness, or that your cough medicine just isn't working, that's worth mentioning. Please check with your pharmacist or doctor before combining them rather than changing anything on your own.

This combo product (sold as Theracaps) packs in acetaminophen, dextromethorphan, phenylpropanolamine, and pyrilamine. The piece that matters here is the acetaminophen (Tylenol), which is partly handled by a liver enzyme called CYP2E1. Lab and animal studies hint that Danshen may slow that enzyme down. In theory, that could let a little more acetaminophen linger in your system.

The good news: this has never actually been reported in real people, and it rests only on test-tube work, so I wouldn't lose sleep over it. Still, since acetaminophen is hard on the liver in large amounts, it's smart not to exceed the dose on the label. If you take Danshen regularly, just give your pharmacist or doctor a heads-up first.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

This combo cold-and-cough product (Theracaps) includes acetaminophen, which your liver partly clears using a process called glucuronidation. Danshen, in lab and animal studies, seems to tug on those same enzymes in both directions, sometimes speeding them up and sometimes slowing them down. The catch is that this has never actually been reported in real people, so we're really talking about a theoretical concern here.

If danshen did slow that clearance, acetaminophen could build up a bit, which matters most for the liver. So I'd just be sensible: don't double up on acetaminophen from other products, go easy on alcohol, and check with your pharmacist or doctor before pairing these, especially if you use danshen regularly in concentrated supplement form.
