Elexacaftor, Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

All three pieces of Trikafta (elexacaftor, tezacaftor, and ivacaftor) are cleared from your body mainly by a liver and gut enzyme called CYP3A4. The concern here is that taking Danshen regularly for a week or two seems to speed that enzyme up. If your body clears Trikafta faster, blood levels can drop, and that may mean it does not work as well at keeping your lungs and digestion in better shape.

This isn't a proven Trikafta interaction, just a reasonable worry based on small studies with a different drug, so I wouldn't panic. But because Trikafta is so important to keep at steady levels, please don't start or stop Danshen on your own. Check with your CF team or pharmacist first.

What Is Elexacaftor, Tezacaftor, and Ivacaftor Used For?

Brand name: Trikafta

Trikafta (elexacaftor, tezacaftor, and ivacaftor) is used to treat cystic fibrosis (CF) in adults and children aged 2 years and older who have a clinical diagnosis of CF and carry at least one CFTR gene variant that is expected to respond to the medication — either based on clinical trial data or laboratory testing.

If a patient's genetic variant is not already known, an FDA-cleared CF genetic test should be used to confirm eligibility before starting treatment.
