Why this applies to Dolutegravir, Rilpivirine

Dolutegravir, Rilpivirine falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the situation with Juluca (dolutegravir plus rilpivirine) and Danshen. Dolutegravir is partly handled by a transporter called P-glycoprotein, which acts like a gatekeeper moving the drug in and out of your body. In one small study, Danshen slowed that gatekeeper down and let more of another P-gp drug build up. So in theory, Danshen could nudge your dolutegravir levels a bit higher.

Neither of these is a prodrug, so higher levels would mean stronger effects and possibly more side effects like headache, trouble sleeping, nausea, or feeling dizzy. The evidence here is limited and the effect was modest, so don't panic. With HIV therapy you really want steady, predictable levels, so please check with your pharmacist or doctor before adding Danshen.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Dolutegravir, Rilpivirine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Juluca is your two-in-one HIV pill, and the rilpivirine part is cleared by an enzyme called CYP3A4. Here's the catch with Danshen: when taken regularly for a week or two, it appears to speed that enzyme up. In a small study it cut levels of a similar CYP3A4 drug by a lot. For you, that could mean less rilpivirine in your blood, and with HIV medicines, low levels are a real worry because the virus can start to slip through and build resistance.

Oddly, a single dose of Danshen does the opposite and raises levels. Either way, this is not a combo I'd guess at on your own. Please check with your pharmacist or HIV provider before adding Danshen, since keeping your HIV treatment working matters most.

Dolutegravir, Rilpivirine also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the situation with Juluca (which combines dolutegravir and rilpivirine) and Danshen. Dolutegravir is partly cleared by a process called glucuronidation, mainly through enzymes in the UGT family. Danshen has shown mixed effects on these enzymes in lab studies, sometimes speeding them up and sometimes slowing them down. Because the lab results pull in both directions, we honestly can't predict which way things would shift in a real person.

The key thing to know: this interaction is theoretical, based only on test-tube research, and has never been reported in people. Still, with HIV therapy, keeping drug levels steady matters, since too little could weaken the medicine. I wouldn't panic, but please run Danshen by your pharmacist or doctor before adding it.
