DOLUTEGRAVIR (Tivicay)

Calcium seems to reduce levels of dolutegravir.
Advise patients to take dolutegravir either 2 hours before or 6 hours after taking calcium supplements. Pharmacokinetic research suggests that taking calcium carbonate 1200 mg concomitantly with dolutegravir 50 mg reduces plasma levels of dolutegravir by almost 40%. Calcium appears to decrease levels of dolutegravir through chelation.

Here's the deal with calcium and Juluca (which contains dolutegravir plus rilpivirine). Calcium can grab onto the dolutegravir piece in your gut and bind it up, a process called chelation. When that happens, less of the medicine gets absorbed. In one study, a single calcium dose dropped dolutegravir blood levels by almost 40%, which is a big drop for an HIV medicine.

Why this matters: keeping HIV virus suppressed depends on steady drug levels. If levels dip too low, the virus has a chance to come back or build resistance. The good news is timing fixes it. The usual advice is to take your Juluca 2 hours before or 6 hours after your calcium. Please check the exact plan with your pharmacist or doctor before changing anything.
