Calcium has a major interaction with Ceftriaxone. Co-administration of intravenous calcium and ceftriaxone can result in precipitation of a ceftriaxone-calcium salt in the lungs and kidneys.

CEFTRIAXONE (Rocephin)
Avoid administering intravenous calcium in any form, such as parenteral nutrition or Lactated Ringers, within 48 hours of intravenous ceftriaxone. Case reports in neonates show that administering intravenous ceftriaxone and calcium can result in precipitation of a ceftriaxone-calcium salt in the lungs and kidneys. In several cases, neonates have died as a result of this interaction. So far there are no reports in adults; however, there is still concern that this interaction might occur in adults.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know with ceftriaxone (Rocephin): this is an interaction about the IV form, not your bones. When ceftriaxone and calcium meet in the bloodstream, especially when both are given into a vein, they can clump together and form tiny solid particles. Those particles can lodge in the lungs and kidneys, which is dangerous. This is why hospitals avoid giving IV calcium (including calcium-containing IV fluids like Lactated Ringer's or IV nutrition) anywhere near IV ceftriaxone.
The most serious cases were in newborns, and some were fatal, so this is taken very seriously in babies. There aren't confirmed reports in adults, but caution still applies. A regular calcium supplement you swallow is a different situation, but always tell your care team what you take so they can space things safely.
