Hypercalcemia can reduce the effectiveness of verapamil in atrial fibrillation. Theoretically, use of calcium supplements may increase this risk of hypercalcemia and reduce the effectiveness of verapamil.

Here is the gist for verapamil: this medicine works by blocking calcium from flooding into your heart and blood vessel cells, which relaxes vessels, lowers blood pressure, and slows down a fast heart rate. The worry with calcium supplements is simple. If you push extra calcium into your system, you could in theory blunt how well verapamil does its job, especially when it is being used to control an irregular rhythm like atrial fibrillation.

I want to be honest about the strength here. This concern comes mainly from lab and animal data, so it is more theoretical than proven in everyday patients. Food amounts of calcium are very unlikely to be a problem. Still, before starting a calcium supplement, run it by your pharmacist or doctor so they can keep an eye on your blood pressure and pulse.

Why this applies to Verapamil

Verapamil falls under CALCIUM CHANNEL BLOCKERS — the group Calcium interacts with.

Intravenous calcium may decrease the effects of calcium channel blockers; oral calcium is unlikely to have this effect.
Intravenous calcium is used to decrease the effects of calcium channel blockers in the management of overdose. Intravenous calcium gluconate has been used before intravenous verapamil (Isoptin) to prevent or reduce the hypotensive effects without affecting the antiarrhythmic effects. But there is no evidence that dietary or supplemental calcium when taken orally interacts with calcium channel blockers.

Here's the good news if you take verapamil for blood pressure, chest pain, or a rhythm problem: a regular calcium supplement or the calcium in your food is very unlikely to cause any trouble. Verapamil works by blocking calcium channels in your heart and blood vessels. The concern on file is purely theoretical, and it really only applies to intravenous calcium given in a hospital, which doctors sometimes use on purpose to push back against verapamil's effects.

Taking calcium by mouth simply doesn't flood your bloodstream the way an IV does, so it shouldn't blunt verapamil or change your blood pressure or heart rate. Bottom line, this is a minor, unlikely concern for swallowed calcium. Still, if you have kidney issues or take several medicines, it's worth a quick chat with your pharmacist before starting any supplement.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
